What to expect from TCM at Bridge Healing Center

Step 1

Intake & Referral

Contact us or receive a referral from your healthcare provider, county, or hospital. We review your eligibility for Medical Assistance (Medicaid) TCM services.

Step 2

Comprehensive Assessment

Your assigned case manager conducts an in-depth assessment of your health, housing, social, and functional needs — in your preferred language.

Step 3

Individualized Care Plan

Together we build a written care plan that identifies your goals, the services you need, and how we'll connect you to them.

Step 4

Referral & Linkage

We make warm referrals — not just a list of phone numbers. We introduce you to providers, attend appointments with you when needed, and follow up.

Step 5

Ongoing Monitoring

We check in regularly to make sure services are working and adjust your care plan as your needs change.

Eligibility

Who qualifies for Targeted Case Management?

TCM is available to Minnesota residents enrolled in Medical Assistance (Medicaid) who have a qualifying diagnosis or condition. You do not need a referral from a doctor to contact us — we'll assess eligibility together.

Common questions about TCM

Is Targeted Case Management covered by Medicaid in Minnesota?+
Yes. Targeted Case Management is a Medicaid-covered service in Minnesota for eligible individuals. Most clients have no out-of-pocket cost. We accept Medica, UCare, UnitedHealthcare (UHC), and other Medical Assistance managed care plans.
How is TCM different from therapy or mental health treatment?+
TCM is not a clinical treatment — it's coordination and navigation support. A case manager doesn't provide therapy; they help you get access to therapy, housing, benefits, and other community services you need. Think of us as your guide and connector.
Can I receive TCM and mental health therapy at the same time?+
Absolutely. In fact, this is common and often recommended. BHC offers both case management and outpatient therapy services, so your case manager and therapist can work as a team.
Do I need a doctor's referral to start TCM?+
Not necessarily. You can self-refer by contacting us directly. We'll assess your eligibility during the intake process.
How long does Targeted Case Management last?+
TCM is an ongoing service — there is no fixed end date. We work with you until you've reached your goals and feel confident navigating services on your own, or as long as you're eligible and the service is needed.
Do you offer services in Somali, Arabic, or other languages?+
Yes. Bridge Healing Center has bilingual staff and interpreters available for Somali, Arabic, and other languages commonly spoken by Minnesota's East African community.

Our team responds within one business day. No prior authorization needed — Medicaid enrollment and a Diagnostic Assessment (within 180 days) are the only requirements to start services.
