Eligibility
Who qualifies?
Requires MN Medicaid enrollment and a Diagnostic Assessment within 180 days. BHC can complete the assessment.
Adults with SPMI
Schizophrenia, bipolar disorder, major depression, PTSD, and related diagnoses.
Children with SED
Severe emotional disturbance affecting functioning at home, school, or in the community.
Complex social needs
Housing instability, homelessness, or co-occurring substance use conditions.
East African & refugee communities
Culturally specific services available in Somali and Arabic.

We accept Medica, UCare, UnitedHealthcare, Blue Plus, PrimeWest, and Hennepin Health Medicaid plans. We can also help uninsured clients apply for Medical Assistance.
