From your first call to your first case management session

Contact us

Call or fill out our contact form. We'll ask for your name, date of birth, and Medicaid ID (if you have it).

We verify your coverage

Our team checks your Medical Assistance enrollment and confirms which managed care plan you're enrolled in.

Intake appointment

We schedule your first appointment — often within a few days. No cost to you, covered by your plan.

Services begin

Your case manager starts working with you right away on your assessment, care plan, and first referrals.

Minnesota Medical Assistance eligibility is broad — including for immigrants, refugees, and low-income adults. We can help you apply for Medicaid and connect you to services while your application is in process.

Insurance questions

Do I need Medicaid to get case management at BHC?+
Medicaid (Medical Assistance) is the primary funding source for our case management programs. However, if you don't have Medicaid, we can help you apply — and we may still be able to connect you to services while you wait for coverage to start.
What's the out-of-pocket cost for case management?+
For most clients enrolled in Medical Assistance managed care plans, the cost is $0. Case management is a fully covered benefit under Minnesota Medicaid. If you have a Medicare Savings Program or spend-down, contact us and we'll explain exactly what to expect.
What if I have Medicare and Medicaid (dual eligible)?+
We accept many dual-eligible plans. Contact us with your insurance card information and we'll verify your coverage.
What if I don't have health insurance?+
We can help you apply for Medical Assistance (Medicaid) if you're eligible. Minnesota has broad eligibility — many adults qualify regardless of immigration status through Emergency Medical Assistance or other programs. Contact us to start the process.
Is your service covered under the MinnesotaCare program?+
MinnesotaCare is a subsidized insurance program, not Medicaid. Targeted Case Management requires Medical Assistance enrollment. If you have MinnesotaCare, contact us — there may be other ways we can help.

Our team responds within one business day. No prior authorization needed — Medicaid enrollment and a Diagnostic Assessment (within 180 days) are the only requirements to start services.
